Summary:
Malignancy constitutes significant morbidity and mortality factor in kidney post-transplantation patients. The incidence of cancer in this specific population increases during recent decades. Malignancies in post-transplanted patients seem to be more aggressive and have worse prognosis compared to general population and they are related mainly to the immunosuppressive therapy which is necessary for the graft survival and the oncogenic viruses.
The aim of this study is to analyze data of the Transplant Unit of “Evagelismos” Hospital regarding malignancy after kidney transplantation and its relationship with specific risk factors.
A retrospective cross sectional study was carried out regarding patients who underwent kidney transplantation during the period 2012-2018. Data collection was done with the use of specific questionnaire that included demographic details and information about patient and family history. Data derived from the questionnaire were crosschecked with details from the files of the Transplant Unit and the results of the biopsies from the Histopathological department of our hospital.
There were collected details about 126 patients (75% men, 25% women) with mean age of 51 years. Mean hemodialysis duration was 5.6 years. Eighteen cases of cancer were found in 17 patients (13.5%) with skin cancer being the most common (38.9%). Men demonstrated 3.3 times greater propability for malignancy compared to women (p=0.162). For every one year added to the patients age cancer propability increases 5% (p-0.054). Patients with positive cancer family history had 3 times greater propability for malignancy (p=0,089 marginally). After multifactorial analysis patients with history of cancer had 15.3 times greater propability of malignancy (p<0.001), whereas patients taking statins demonstrated 4.5 times greater propability of cancer after the kidney transplantation (p=0.021).
Long waiting duration for the kidney transplantation reflects the scarcity of grafts in our country. Knowledge about malignancy incidence after kidney transplantation ant its relationship with different risk factors provides us the possibility to follow up and care this vulnerable population with greater effectiveness.
