Summary:
BACKGROUND: ERAS program is a perioperative protocol aiming patients’ faster recovery. It is intended to alter human body’s pathophysiological and psychological reaction to the stress of a surgical intervention.
METHODS: This research is a case series study of patients that underwent elective colectomies for malignant disease and were treated according to the ERAS protocols.
After the patient’s admission, thorough briefing takes place. The patient receives normal food intake the day prior to surgery and no or mechanical only bowel preparation. A carbohydrate rich solution is given 3 hours before surgery. Chemoprophylaxis is administered and the patient receives 2 more doses of antibiotics post-surgery. During the procedure, short-acting anaesthetic drugs are being used. Patient is mobilized just hours after surgery and IV fluids overload is avoided. Food intake is initiated during the day of the procedure. Patient is discharged when complete mobilization is achieved, after having passed flatus and if assessed being in good overall condition.
For a superior outcome evaluation, we compared patients of the ERAS group with patients that underwent the same surgical procedures, but were treated according to more traditional algorithms.
DISCUSSION: Our research depicted the safety and superiority of enhanced recovery protocols. Fewer postoperative complications were recorded, earlier food intake, faster mobilization and overall recovery, shorter hospital stay and thus lower hospitalization cost. On the other hand, no unexpected complications or unwanted events were reported.
CONCLUSION: The enhanced recovery protocols offer a new approach in surgical patient management, which improves the patients’ experience minimizing stress and accelerating rehabilitation.
