Summary:
Introduction: According to the ECDC 2017 Annual Epidemiological Report, 11.787 new cases of in hospital - acquired infections were recorded in ICU patients from which 5.298 were cases of bloodstream infections and the 36.5% were associated with the use of cental - line catheter.
In Greece, it is estimated that more than 4.41% of pediatric patients will
develop some type of infection related to the Central Venous Lines.
Purpose: The purpose of this study is to investigate the knowledge,
the attitudes and compliance of pediatric oncology nurses with
the current guidelines concerning the prevention of bloodstream infections that are
related to central-line venous catheters (CLABSIs) .
Sample-Methodology: The sample of the study was 43 Registered Nurses working in pediatric oncology departments and Oncology
Outpatient Clinics of Attica. To collect the data,
specially designed anonymous questionnaire was used after extensive
review of relevant literature.
Results: A total of 40 female and 3 male nurses participated
with an average age of 41.31+-7.63 years. The majority of participants (N =40,
93.0 %) were university graduates of AEI-TEI where they worked as nurses in
pediatric clinic-ward (N =34, 79.1%), 14.0% as supervisors (N=6) and 7.0%
as nursing assistants (N=3) with an average professional experience of 13.44+-8.70 years.
A large number of nurses answered correctly the questions regarding the flushing of the
lumen of the catheter with saline after drug or fluid administration , the
use of gauze or a sterile transparent pad at the catheter entry point, the
changing of the infusion devices every 72 hours and avoiding the use
of antibiotic ointment at the catheter entry point, with rates ranging from 51.2% to
97.7%. Poor knowledge were observed in the questions regarding
the use of Hydrogen Peroxide as an antiseptic factor in the application of proper hand hygiene, the frequency of changing the
patches, but also the period of time when it is recommended to change the intravenous
infusion device after administration of blood, its derivatives and colloidal solutions. During the statistical analysis it was observed that the nurses who gave
correct answers to all the questions that involved knowledge about the major
recommendations were more likely to know that the use of Peroxide of Hydrogen is not recommended as an antiseptic of the entry point of the CLC (p=0.003), as well as
daily use of anticoagulants (p=0.020). In addition, it was observed that the
nurses who were aware of the guidelines and practices
based on indications worried more about the risk of transmission of infection against
the handling of the CLC (p=0.000).
Conclusions: According to the scientific results, the nursing staff
possesses an adequate level of knowledge about the prevention of CLABSIs. Nevertheless, there is a need for continuing education and
staff training in specific practices related to the handling of
CLC, thereby contributing to the optimal provision of quality care to patients.
