Summary:
Introduction: Colloid cysts of the 3rd Ventricle have successfully been treated using both the microscopic and the endoscopic approach. Despite the reported advantages and disadvantages of each technique, there is still lack of consensus as to the optimal operative strategy. We conducted a systematic review of the literature on the two techniques to compare their results. Materials and Methods: A search was conducted on the PubMed database for published studies on the surgical management of 3rd ventricle colloid cysts. Relevant articles were identified, each article’s quality was assessed and data on the surgical approach and outcomes were extracted.
Results: The GTR rate was higher and the recurrence rate was lower in the microsurgical arm in all but one studies. The length of operation and duration of hospitalization was shorter in the endoscopic arm in almost all studies. Mortality rate tends to be higher using the microsurgical approach, while the endoscopic approach tends to have higher reoperation rates. Conclusions: Microscopic resection exhibits higher GTR rates, lower recurrence rates and fewer reoperations than the endoscopic approach, but higher mortality rate and higher rate of shunt dependency. Endoscopic resection is linked with shorter durations of operation and hospitalization.
