Summary:
Introduction: Coronary artery disease is one of the most common causes of
morbidity, mortality and disability worldwide. Multiple etiological factors leading to the
development of the disease have been identified, and the diagnosis is based on specific
criteria. The therapeutic approach may be either conservative or cardiac surgery may be
performed. The length of stay in the ICU, intubation, and survival depends on many
different factors.
Aim: The aim of this study was to investigate the factors responsible for prolonged
intubation of cardiac surgery patients.
Material and Methods: The study sample was retrospectively 2,136 cardiac
surgery patients who were hospitalized at Healthia Hospital between 01.01.2022 and
31.12.2022. Data were collected from patient records. Statistical t-test, and x2-test were
applied, and a 5% significance level was considered statistically significant.
Results: Most of the sample of 1,929 patients, 90.3% were male. The age of the
sample in 15.6 was less than 55 years, in 29.2% was 56 to 65, in 36.6% 66 to 75 and over
76 years was 18.6%. 46.9% of the sample suffered from diabetes, hypertension was
reported by 86.9%, smokers were 55.0%,ejection fraction below 40 was present in 31.3%
of the sample. The highest percentage 70.8% had three or more grafts. More than 48
hours of intubation were in older subjects (69.9 vs 65.9 years), (p=0.006), who had lower
ejection fraction (37 vs 47), (p<0.001), had more grafts (3.14vs2.92), (p=0.007), more
aortic block time, (p<0.001) and more mechanical ventilation time, (p<0.001), with
peripheral vascular disease p<0, 001, with emergency admission p<0.001, with intraaortic pump application before surgery p=0.049, with damage to the main left artery
p<0.001, who presented with myocardial infarction p<0.001, with damage to more than
four vessels p=0.037, who were fitted with an intra-aortic pump p<0.001, and those who
died p<0.001.
Conclusions: Factors associated with prolonged intubation and poor outcome are
demographic and clinical both preoperative and intraoperative and postoperative.
