Summary:
Storing platelets at room temperature with a maximum storage time of 5 days, although optimizing platelet viability after transfusion, limits ef-fective inventory management and availability. Modifications in platelet preservation may increase the effectiveness of bleeding control, while making imperative to find novel methods of preserving platelets for an extended period of time.
The use of new preservation methods, such as platelet cryopreservation, provides the possibility to preserve the functionality of platelets while simultaneously reducing adverse transfusion reactions. Cryopreserved platelets are suitable for both civilian and military use, particularly for the treat-ment of acute conditions associated with massive bleeding when there is no permanent or sufficient supply of platelet products.
Cryopreserved platelets act as a substitute for conventional platelets by ehnancing the blood clotting mechanism in patients with platelet deficiency. They can also be used in other cases, such as autologous donations, rare or HLA / HPA compatible platelets as well as in a wide range of cases beyond transfusions. Cryopreserved platelets have a long shelf life and can lead to an increase in stocks without significantly increasing the cost.
The purpose of this paper is the presentation of cryopreserved platelet properties and their therapeutic role in clinical practice, as well as the investigation of the most recent data and perspectives.
Future efforts should be directed to the selection of a strictly defined cryopreservation protocol, with the aim of designing more reliable clinical trials with more comparable results. Until then, safety and efficacy testing is required in every area where cryopreservation is required. Finally, it should be noted that the transfusion of cryopreserved blood platelets has not been sufficiently investigated and more studies are required for the safety of the procedure and the resolution of the problems involved in the use of cryopreserved blood platelets in clinical practice.
