Optimal blood use in the blood bank unit of a tertiary public hospital and strategies for improvement: a study of vascular surgery clinic patients
Summary:
The present study is focused on the optimal blood management in the Blood Bank Unit, Red Cross “Korgialenio–Benakio” General Hospital, Athens, Greece. Specifically, the aim of this study was the determination of the potential corrective actions during vascular surgeries and the development of a Maximum Surgical Blood Ordering Schedule (MSBOS).
Τhe data from 218 emergency and elective vascular surgeries performed from January to December 2022 in 182 patients (149 males, 33 females), were used and the Crossmatch to Transfusion Ratio (CTR), Transfusion Probability (T%), Transfusion Index (TI), Blood Utilization (BU) and MSBOS were calculated.
The results of the present study revealed that in the majority of vascular surgeries, the units of Packed Red Blood Cells (PRBC) were rationally managed, as indicated by the values of the CTR, T% and TI indexes. In particular, for the majority of the surgeries, CTR values were below 2.5, T% values were above 30% and TI values were above 0.5. Regarding the BU index, its mean value indicates that approximately 55% of the PRBC units which were crossed-matched were not used.
Regarding the proposed reference MSBOS [British Committee for Standards in Haematology (BCSH), Blood Transfusion Task Force], it is worth noting that it is available for a limited number of vascular surgeries. The data of the present work could be used for the development of a local reference MSBOS, which will be applied in vascular surgeries.
