Chronic spontaneous crticaria and NSAID hypersensitivity
Summary:
Introduction: Chronic Spontaneous Urticaria is characterized by the intermittent development of hives, angioedema, or both, for more than 6 weeks. The disease is caused by mast cell degranulation through underlying pathophysiological mechanisms, which, although not fully elucidated, involve two types of hypersensitivity reactions: type I (autoallergy mediated by IgE antibodies against endogenous antigens), and type IIb (the presence of IgG antibodies against FcεR1 or IgE, which induce mast cell degranulation).
Non-Steroidal Anti-Inflammatory Drugs are a widely used large family of drugs, known for their anti-inflammatory, analgesic and antipyretic effects. They are the most frequently reported cause of drug hypersensitivity reactions with clinical manifestations that most often concern the respiratory system and the skin as target organs. Exacerbation of CSU by NSAIDs is described as NECD (NSAID Exacerbated Cutaneous Disease) and is caused through many signaling pathways related to the NSAIDs’ mechanism of action.
Methods: Using the data of 173 patients with CSU of the U.G.H. «ATTIKON», registered in an international patient registry, 12 qualitative and quantitative variables were correlated with the NECD patients and the other CSU patients of the population.
Results: In the study population consisting of 173 CSU patients with a mean age of 43.8 ± 17,8 years, of which 70.5 % were female, 23 patients (13.3%) with NECD were found. Among the evaluated parameters, it was found that NECD presented a statistically significant correlation with female gender, mean age (51,4 ±19,5), and the co-existence of basopenia (≤0.01x109/L) in CBC.
Conclusions: Using the above statistically significant correlations, a specific profile of a patient with NECD is formed concerning a woman in advanced middle age with basopenia.
