Summary:
Introduction: Multiple Sclerosis is an autoimmune disease, which manifests mainly at young ages (20 - 40 years) and with greater frequency in the female population. The immune system targets myelin, the substance that surrounds the nerve axons of the brain and spinal cord. Anxiety and depression have a harmful effect on quality of life in patients with MS.
Aim: This study aim is to verify the relationship of depression and the quality of life with the degree of disability in MS.
Method: This research is observational, descriptive, and cross-sectional study. The initial study registration contained demographic data. Next, important information related to the patient's medical condition such as the age of onset of the disease, its duration, the number of relapses and hospitalizations in the last year. Information regarding the course and form of the disease was also obtained. During the evaluation, symptomatology was assessed using the Beck Depression Inventory (B.D.I.), the SF36v2 Questionnaire, and the Extended Disability Status Scale (EDSS). In the analysis, Pearson's X2 criterion and the exact test were used where necessary. After the normality test (Shapiro – Wilk) non-parametric criteria were applied to compare the median values (Mann-Whitney and Kruskal-Wallis with post-hoc test the sidak). Between continuous variables correlation was investigated with Spearman's correlation coefficient. Reliability testing was done with the internal consistency coefficient Cronbach's alpha. Multiple logistic and linear regression were also used. Analysis was performed with the statistical package STATA v12.1.
Results: This study was carried out at the General Oncology Hospital "Agioi Anargyroi" in the period from January to September 2022. 101 people participated, of which 60.4% were women. The predominant age group was that of 50-64 years (40.6%) and the majority were married (56.4%). Gender does not seem to differ statistically in relation to age (p=0.675), years of symptoms (p=0.061), treatment regimen (p=0.367), number of changes in treatment regimens (p=0.088) and number of relapses since the onset of the disease (p=0.283). The type of the disease seems to differ statistically in relation to age (p<0.001). Using the Spearman Correlation Coefficient it was found that there is a strong positive correlation between the degree of disability based on the EDSS scale and depression based on the Beck scale (rs=0.8763 and p<0.001). The physical functioning factor of the SF36v2 scale has a strong negative correlation with the degree of disability (rs=-0.8147 and p<0.001) and depression (rs=-0.7408 and p<0.001), the physical role factor of the SF36v2 scale has a strong negative correlation with the degree of disability (rs=-0.7984 and p<0.001) and the existence of depression (rs=-0.7269 and p<0.001), while the vitality factor of the SF36v2 scale has a moderate to strong negative correlation with the degree of disability (rs=-0.6324 and p<0.001) and the presence of depression (rs=-0.6939 and p<0.001). It was also found that the general health factor of the SF36v2 scale does not seem to be correlated with the degree of disability (p=0.5255) and the presence of depression (p=0.5656).
Conclusions: Depression and quality of life in MS patients correlate with the degree of disability. Based on this, the implementation of programs aimed at preventing depression and improving the psychosocial support of patients is considered necessary.
