Summary:
The purpose of this study was to investigate the safety and efficacy of microwave ablation (MWA) for centrally located T1a (<4cm) kidney tumors. A review of the available literature on the use of MWA in the treatment of kidney tumors was conducted, with a focus on published studies concerning the safety and efficacy of MWA in treating centrally located T1a renal carcinomas. For patients diagnosed with localized T1a renal carcinoma, the treatment goal is to preserve normal renal parenchyma while achieving local cancer control. Comparisons between MWA and partial nephrectomy, as well as between MWA and the other two clinically available ablation techniques, radiofrequency ablation (RFA) and cryoablation (CA), showed similar outcomes in disease control, complications, cancer-specific survival, and primary efficacy in treating T1a renal tumors. Over the past decade, MWA for the treatment of T1a RCC has made significant progress, with a local disease progression rate of less than 5.0% and technical efficacy approaching 100%. The latest microwave systems are more powerful, perform better, and require shorter ablation times, thus presenting a lower risk of thermal injury to the collecting system. Some data suggest that the tumor's location and proximity to the pelvicalyceal system do not impact the technical success of MWA, its safety, complication rates, or oncological outcomes. The use of adjunct techniques, such as the injection of cold fluid into the pelvicalyceal system, can be employed for tumors with an endophytic location to reduce the likelihood of complications. Image-guided percutaneous MWA (using CT or ultrasound) is a safe and effective treatment option for T1a kidney tumors, with high technical success and low recurrence and complication rates, comparable to or even better than those of CA and RFA.
