Summary:
Introduction: The diagnosis of asthma in preschool children is a challenge for the medical community. In 2019, the guidelines were revised with the aim of standardizing spirometry in preschool children.
Purpose: To investigate the feasibility of performing spirometry without technical errors in children aged 3 to 6 years old and the effect of educational intervention. Sub-objectives were the comparison with pulse oscillometry and the investigation of child and family factors influencing their outcome.
Methodology: 133 children who visited the pediatric pulmonology unit of a public hospital and who had not attempted spirometry before. Initially, impulse oscillometry was performed and then the control group received oral training on the spirometry procedure, while the intervention group watched the training video. All children were given a growing whistle to familiarize them with the technique. The study period is from 08/05/2019 to 11/03/2020 and from 21/03/2022 to 22/01/2024 due to the covid-19 pandemic. The statistical analysis of the data was done using IBM SPSS Statistics 26.0 software.
Outcomes: The overall success rate for spirometry was found 74.4% and for impulse oscillometry 75.9%. Sex, presence of disease, history of asthma and educational level of the parents were not related to the outcome of the examination. The educational intervention increased the probability of successful spirometry by 3.85 times and the number of accepted attempts and reduced the total time to perform the test. The median number of attempts was found to be 9. Children with more sociability were found to be more likely to have at least one acceptable attempt, while children with hyperactivity and more difficulties according to the SDQ needed a greater number of attempts.
Conclusions: It is feasible for preschool children to perform technically accepted spirometry. The educational intervention has a positive effect on successful spirometry.
