Summary:
Ιntroduction: Nesiritide was compared to nitroglycerin on patients suffering from acute heart failure in need of vasodilatory treatment. Outcomes of interest were treatment’s renal effect, which included serum creatinine, estimated glomerular filtration rate , Cystatin C, beta 2 microglubin , worsening renal function, rehospitalization and death rate.
Purpose: To estimate the effect of vasodilator treatment (recombinant human BNP or nesiritide versus nitroglycerin) on renal function in patients with acute decompensated heart failure (ADHF).
Methods:A systematic review of English and non-English articles using MEDLINE and CENTRAL published up to November 2020 was performed. Additional study was identified through other sources. Search terms included acute heart failure ,nitroglycerin, glyceryl trinitrate, nesiritide , recombinant human brain natriuretic peptide, rhBNP. Randomized controlled trials (RCTs) concerning human adults and comparing nesiritide versus nitroglycerin with outcomes of interest were identified.Independent extraction of articles by 2 authors using predefined data fields, including study quality indicators was performed.
Results: Three RCTs met our inclusion criteria. Two of them and most reliable trials favor the superiority of nesiritide, whereas one favors nitroglycerin.
Conclusions: The evidence so far remain conflicting and not sufficiently robust to determine the comparative effect between nesiritide and nitroglycerin.
