Interpretation and Supportive Techniques in Various Models of Psychoanalytic Psychotherapy for Borderline Patients
Summary:
Over time, there has been a significant increase in the number of individuals who do not fit into traditional categories of mental disorders such as psychotic or neurotic disorders. This creates an urgent need for the development of specialized therapeutic approaches for Borderline Personality Disorder (BPD).
Borderline Personality Disorder (BPD) is a complex and serious psychiatric condition characterized by emotional instability, difficulties in interpersonal relationships, and self-destructive tendencies. This thesis focuses on exploring and reviewing the use of psychoanalytic interpretations and supportive psychoanalytic techniques for treating borderline patients. Psychoanalytic interpretation involves analyzing transference and countertransference, interpreting defense mechanisms, analyzing repeated patterns of behavior, and exploring early experiences that shaped the patient's psychological structures. On the other hand, supportive psychoanalytic techniques include establishing a safe and supportive therapeutic relationship (supportive alliance), enhancing mentalization capacity, providing structure and boundaries, interpreting emotional reactions, clarifying and reflecting, and analyzing transference and countertransference.
Additionally, significant emphasis is placed on the importance of the therapeutic framework in treating borderline patients. Borderline patients often struggle with accepting the framework due to lack of an internal sense of safety. Treatment necessitates creating a secure space where the patient can explore insecurities and develop new emotional processing capabilities. The importance of specialized therapeutic teams in comprehensive care and addressing the complexities of borderline personality is examined. Interdisciplinary collaboration among specialists enhances understanding of the patient and contributes to developing healthier behavioral models and improving their quality of life.
The paper also briefly but comprehensively reviews the psychopathology of borderline personality disorder, focusing on understanding the disorder and therapeutic approaches, as well as evaluating the effectiveness of psychodynamic psychotherapy for patients with this disorder.
The models presented include Kernberg's Transference Focused Psychotherapy (TFP), Fonagy and Bateman's Mentalization-Based Treatment (MBT), Bion's containment model, and the fundamental principles of therapeutic approaches as analyzed by Gabbard.
The conclusion drawn is that the combined use of psychoanalytic interpretations and supportive techniques can offer deep insight into the internal conflicts of borderline patients, helping them develop better self-awareness, improved emotional regulation, and healthier interpersonal relationships. The stable and confidential therapeutic relationship, reinforced and supported by the supervisory and interdisciplinary team, forms the cornerstone for successfully managing this demanding disorder.
