Anxiety, depression, stress and suicidal ideation in hemodialysis patients
Summary:
Introduction: The hemodialysis in the context of Chronic Kidney Disease affects the psychology of patients and there are significant indications that it causes anxiety, depression, stress and suicidal ideation.
Purpose: To investigate the association of depression, anxiety, stress and suicidal ideation with hemodialysis patients.
Material - Method: The study sample consisted of 104 patients undergoing hemodialysis in public and private Kidney Hemodialysis Units in Athens, during the period from 1/4/2024 to 30/6/2024. The data collection was implemented using: a) a questionnaire of individual and demographic data; b) the DASS21 questionnaire, which measures the emotional dimension of depression, anxiety and stress; and c) the RASS questionnaire, a self-report instrument for suicidal ideation.
Results: Among the 104 hemodialysis patients who participated in the study, 65.4% were male. The sample’s middle age was 65.9 years old, and almost all of the participants (96.2%) were Greek citizens. According to the data analysis, 11.6% showed severe to very severe symptoms of depression, 17.3% severe to very severe levels of anxiety and 7.8% very severe levels of stress. More intense suicidal ideation was related to low income as well as to the use of a hemodialysis catheter, which is a great influence of the disease on life and increased levels of anxiety and stress. It was also observed that the time of inclusion in hemodialysis from 6 months to 3 years showed higher levels of depression. Finally, no differences were observed in the occurrence and intensity of symptoms of depression, anxiety, stress and suicidal ideation in patients of public units compared to private ones.
Conclusion: It was found that the mental health of patients undergoing renal replacement therapy with hemodialysis is moderate to quite good. The importance of improving the clinical recognition of early symptoms, the implementation of routine screening for psychological distress, the early recognition of suicidal ideation and effective mental health care are important prevention approaches.
