Summary:
Ιntroduction: CPAP (continuous positive airways pressure) is the treatment of choice to treat moderate and severe obstructive sleep apnea. The titration can be done manual or auto and the study can be overnight or only at the second half of the night (split night). Until today the number of studies comparing the two titration methods is limited.
Aims and objectives: Τo estimate if auto titration is as effective as manual to address the respiratory events in both overnight and split night titration and if it is tolerated by patient during titration.
Patients and methods: 100 patients with sleep apnea syndrome (OSA) participated to a titration study. In 50 patients the titration of CPAP was done manually and the other 50 automatically. In each group, 25 patients were subjected to split night study and 25 in overnight titration. At the end of titration, the patients completed a questionnaire about the night of the study.
Results: There is no statistically significant difference between auto and manual titration in respect the elimination of respiratory events, the architecture and the efficiency of sleep both overnight and split night titration. The pressure (P95) applied to Automatic titration was higher (10,7 ± 2.2) from the maximum pressure of the manual (8.1±2,7) in both overnight (auto titration 11.0 ± 2,78, manual titration 7,9±2,7) and a split night titration (auto titration 10.5 ± 2.4, manual titration 8,2±2.7). Also 64,5% of the patients with auto titration reported that their sleep was much better than usually had while with manual titration 46,9% of the patients reported better sleep respectively.
Conclusions: From our finding, it seems that the auto titration is as effective as manual to eliminate the respiratory events both overnight titration and split night However auto titration uses higher pressures to eliminate apneas - hypopneas. Moreover the group of patient who slept with auto titration reported better sleep.
