Summary:
Introduction: Measurement of cardiac enzymes postoperatively is a tool for the
diagnosis of postoperative complications that may lead to prolonged ICU hospitalization
and for patient prognosis.
Aim: The aim of the present study was to investigate the relationship between
postoperative troponin T levels in patients after cardiac surgery and the length of stay
in the ICU
Material and Methods. The sample of the study consisted of 120 patients who
had undergone cardiac surgery with extracorporeal circulation at G.H.A. "O
EVAGGELISMOS” All patients have undergone open heart cardiac surgery. Data were
collected from clinical and laboratory examination of the patients. Data regarding
demographic and clinical characteristics were also recorded. t-test, anova and x2-test
statistical tests were applied. Statistical significance was set at p<0.05
Results: Of the 120 patients in the sample, 92 (76.7%) were male. The age of
the sample in 14.2% ranged in the age range 34-55 years, 46.7% in the 56-70 years
age range and 39.2% were more than 71 years old. 57% of the sample suffered from
arterial hypertension, diabetes mellitus and dyslipidemia. 66.7% stayed up to 2 days in
ICU, while 33.2% stayed more than 2 days. In terms of outcome, 5 subjects (4.2%) of
the total sample died. High troponin values preoperatively were associated with
prolonged ICU stay (p=0.007) as those patients who had high troponin on postoperative
day 1 (p=0.053), postoperative day 3 (p=0.028) and postoperative day 4 (p=0.044)
remained in the unit for more days. More days of ICU stay were spent in the ICU by
men (p<0.002), those who came with emergency admission (p=0.016), those who
developed postoperative complications (p=0.039) and those who died (p=0.039).
Preoperative troponin was positively associated with age (p=0.011) and negatively
associated with CE (p=0.025). Postoperative troponin was positively associated with duration of surgery, out-of-hospital time and ischemia time (p<0.005) and negatively associated with EF (p<0.005)
Conclusion: We found that high troponin postoperatively is directly associated
with prolonged ICU hospitalization beyond the planned time (48 hours) and affects the
patient's prognosis. Factors such as surgery, time of extracorporeal circulation and
ischemia and ejection fraction affect troponin value postoperatively.
